Replacement Document

If any of the following documents have been lost, stolen or destroyed, an Application for RF134 Replacement Document form must be completed by the registered owner.  It should be stamped and signed at a Garda Station and submitted to Leitrim Motor Tax Office with the relevant fee.

  • Logbook (Brown)                                       €12
  • Vehicle Registration Certificate                 €12
  • Tax Disc                                                      €6
  • Trailer Licence Card                                    €6

If the registered owners address has changed a utility bill, as proof of address, dated within the last 3 months must be submitted along with the completed form.

Vehicle Registration Certs are posted directly from the Department of Transport, Shannon, Co. Clare and can take between 5-7 days to issue.  All other replacement documents issue from Leitrim Motor Tax Office.

Please Note: Where a tax disc is lost in the post, a replacement disc is free of charge, provided the application is received no earlier than ten working days and no later than 30 working days from the date of issue of the disc. Applications received outside these times will be liable to a fee of €6.
